Jonelle Moore
As the founder of Arbor Day Guardian Care my desire is to strategically enhance the pattern of care for those with declining health. The idea came to be from experience in the community and the growing education from various community-based relationships. Understanding the frustrations from both sides of providing care and receiving it allows perspective into how healthcare lacks in its ability to follow up with patient needs. In that understanding, it became clear that there is a gap in care that cannot be filled by primary care nor any other facility-based provider. As many factors stand in the way of providers wanting to fill the need and being capable of filling the need, the Continuity of Care Program was created to do just that.
As the Executive Director at Arbor Day Guardian Care, I am responsible for initiating, researching, planning, and developing goals while providing support to the community. Arbor Day Guardian Care’s mission is to support providers in caring for their patients during the transition from a facility or in routine care. The mission is to help providers reduce readmissions and patient/caregiver feel more in control of their care, and I aid in coordination of care, in-home follow up visits (between provider visits), resolve patient conflict, provide community-based resources, housing, and provide guardianship services.
I earned a Bachelor’s in Healthcare Administration with a focus in Leadership at Capella University and am currently pursuing a Master’s in Business with a focus in Healthcare.
Aziz Issa
With a background in healthcare, I have supported the development of Arbor Day Guardian Care to engage with the community. Having experience in various healthcare settings and an education with a focus on disparities allowed for the needed experience to advocate on behalf of the non-profit organization. Along with a desire to change the way providers and the community connects with those in need, particularly the elderly and disable, I have dedicated my work to ensuring I help and connect care for as many lives possible.
As the Director of Philanthropy at Arbor Day Guardian Care, I am responsible for cultivating donors, developing outreach strategies, and overseeing aspects of events and other elements. My focus is to plan the budget and allocate funds to different areas, help update staff on all areas of the mission, and help raise the profile of the charitable giving. Arbor Day Guardian Care’s mission is to support providers in caring for their patients during the transition from a facility or in routine care. The mission is to help providers reduce readmissions and patient/caregiver feel more in control of their care.
I earned a Master’s in Business from Eastern Michigan University.
