Our Mission
ADGC desires to support providers in caring for their patients during the transition from a facility to routine care. The mission is to help providers reduce readmissions and patients/caregivers feel more in control of their care. Patient readmissions have been a topic in health care for many years. The Hospital Readmission Reduction Program (CMS) was created to change how patients have been treated whilst in the facility and the follow-up after the admission.

The vision of ADGC is to support the quality of life for every patient and the quality of care given by every provider through coordination and patient supervision.
Who We Are
Arbor Day Guardian Care is federally recognized 501(c)3 with the IRS, registered in the State of Michigan, founded in 2021. The idea came to be from experience in the community and the growing education from various community-based relationships. Understanding the frustrations from both sides of providing care and receiving it allows perspective into how healthcare lacks in its ability to follow up with patient needs. In that understanding, it became clear that there is a gap in care that cannot be filled by primary care or any other facility-based provider. As many factors stand in the way of providers wanting to fill the need and being capable of filling the need, the Continuity of Care Program was created to do just that.
What Makes Us Unique
Patient readmissions have been a topic in health care for many years. The Hospital Readmission Reduction Program (CMS) was created to change how patients have been treated whilst in the facility and the follow-up after the admission. CCP can address patient/family concerns in a timely manner by creating a base for initial communications and decreasing the number of incoming calls for complaints or grievances. Staffing that provides needed home wellness checks for patient ongoing needs and/or concerns and enhances the patient quality of care by strengthening the daily communication.
